Response choices are shown in a list; multiple answers can be chosen. The checkbox collects multiple selected responses from a list of pre-generated text or value responses.

When creating a checkbox question type, select “Checkbox” “Checkbox” from the list of available question types and fill in the remaining required fields.

Fill in the required text and value fields. If more rows are needed, simply click “Add “Add a Row” Row” and a new row will appear with empty fields to be filled.
