Response choices are shown in a list; multiple answers can be chosen. The checkbox collects multiple selected responses from a list of pre-generated text or value responses.

When creating a checkbox question type, select

“Checkbox” from the list of available question types and fill in the remaining required fields.

Row” and a new row will appear with empty fields to be filled.
